You must submit your online claim form or mail your completed paper Lincare Settlement Claim Form with a postmark no later than Monday, April 15, 2024.
Lincare Settlement Claims Form:
This Claim Form is intended to apply for benefits under the Settlement with Lincare Holdings for claims involving the alleged unauthorized disclosure of protected health information and personally identifiable information. You must provide the information requested in this Claim Form for each applicable Claim to receive payment as part of this Settlement.
